If you're a new client, please complete the Intake Packet through the client portal at least 48 business hours (M-F 8a-5p) in advance of  your session. Paper versions of the intake paperwork are available below, however, if you complete paper versions, they must be submitted to your clinician at least 48 business hours in advance.
